Statement of Patient Rights


Reedsburg Physicians Group, S.C. respects the right of the patient, recognizes that each patient is an individual with unique health care needs, and because of the importance of respecting each patient's dignity, provides considerate, respectful care focused upon the patient's individual needs.

Reedsburg Physicians Group, S.C. affirms the patient's right to make decisions regarding his or her medical care, including the decision to discontinue treatment, to the extent permitted by law.

Reedsburg Physicians Group, S.C. assists the patient in the exercise of his or her rights and informs the patient of any responsibilities incumbent upon him or her in the exercise of those rights.

The policies and procedure that guide Reedsburg Physicians Group's interaction with and care of the patient demonstrate its recognition and support of patient rights.

No listing of patient rights can assure the respect of those rights. It is the intent of this document to ensure that the organization's interaction with and care of the patient reflects concern and respect for the rights of the patient.

Reedsburg Physicians Group, S.C. has established its policies and procedures to establish and support the following patient rights:

Reasonable Access to Care

In response to the patient's request and need, the organization provides care that is within its capacity, its state philosophy and mission, and applicable law and regulation. When the organization can not meet the request or need for care because of a conflict with its philosophy or mission or incapacity to meet the patient's needs or requests, the patient may be referred to another facility when medically permissible. Such a referral should be made only after the patient has received complete information and explanation concerning the needs for and alternatives to such a referral.

Considerate Care that Respects the Patient's personal Value and Belief Systems

The provision of patient care reflects consideration of the patient as an individual with personal value and belief systems that impact on this or her attitude toward and response to the care provided by Reedsburg Physicians Group. The policies and procedures that guide patient care include recognition of the psychosocial, spiritual beliefs and procedures allow the patient to express spiritual beliefs and cultural practices that do no harm others or interfere with the planned course of medical therapy for the patient.

Informed Participation in Decisions Regarding His or Her Care

The quality of patient care is enhanced when the patient's preferences are incorporated into plans for care. The process by which care and treatment decisions are made should elicit, respect, and incorporate the patients' preferences. Sound medical judgment should be provided to the patient or the patient's surrogate decision maker for the informed decision-making. The patient should be given a clear, concise explanation of his or her condition and of any proposed treatment(s) or procedures, problems related to recuperation, and the likelihood of success. Information should also be provided regarding any significant alternative treatment(s) or procedure(s).

The patient is responsible for providing, to the best of his or her knowledge, accurate and complete information about present complaints, past illnesses, hospitalization, medications, advance directives, and other matters relevant to his or her health and care. The patient is also responsible for reporting whether he or she clearly comprehends a contemplated course of action and what is expected of his or her.

Participation in the Consideration of Ethical Issues That Arise in the Provision of His or Her Care

Health care professionals provide patient care within an ethical frame work established by their profession, the medial group, and the law. The health care professional has an obligation to respect the view of the patient or the patient's designated representative when ethical issues arise during the patient's care.

Personal Privacy and Confidentiality of Information

The patient has the right to be interviewed, examined, and treated in the surroundings designed to give reasonable visual and auditory privacy. The patient also has the right to have access to his or her medical record and to have his or her medical record read only by the individuals directly involved in his or her care, or by individuals monitoring the quality of the patient's care, or by individuals authorized by law or regulation. (Other individuals may read the medical record only the patient's written consent or that of a legally authorized designated representative.)

Designation of a Representative Decision Maker in the Event that the Patient is Incapable of Understanding a Proposed Treatment or Procedure or is Unable to Communicate His or Her Wishes Regarding Care

While the patient is recognized as having the right to participate in his or her care and treatment to the fullest extent possible, there are circumstances under which the patient may be unable to do so. In these situations, the patient's rights are to be exercised by the patient's designated representative or other legally designated person.
