Contracted Insurance Plans


We file claims with insurance companies that do not require contract participation. If your insurance carrier is not listed, and you would like to know if services provided by Reedsburg Physicians Group will be covered, please contact your insurance plan's customer service department. They can provide you with the most accurate coverage information. If further assistance is needed, please contact the Patient Accounts Department at 608-524-6477.

Insurance Claims

  • Reedsburg Physicians Group will file claims with your primary and secondary insurances providing you have supplied us with your current insurance information.

  • It is essential that you inform our office if you have any changes in your insurance coverage so we can process your insurance billing promptly and efficiently.

  • Please bring your insurance card with to each visit. It is your responsibility to be aware of your insurance plan's specific coverage.

  • If you are a member of a PPO plan make sure that Reedsburg Physicians Group is a participating provider of your plan.

  • If you are a member of a HMO plan, make sure that Reedsburg Physicians Group is listed as your primary care clinic.

Workmen's Compensation

Employees who schedule appointments for work injuries or illnesses should notify the scheduler of their work injury status. When registering for the appointment please inform the receptionist the visits charges are to be filed with your employer's workmen's compensation carrier. You will be given a Work Injury Form to complete. This process will enable us to process your claim accurately and efficiently.
